To Qualify for the Medical Supplies Program a person
must be eligible for services and able to provide documentation
of eligibility. Program eligibility is generally based on various
income, age, and residency criteria established by our funding
sources. Please call for clarification of eligibility information
and eligibility updates for each program.
YOU MAY QUALIFY FOR PRESCRIPTION
ASSISTANCE IF
• You have been diagnosed with chronic lung disease.
•
You have a current doctor’s prescription related to your
chronic lung disease.
• Your total household income is within federal guidelines
for low-income assistance.
• You are having a problem with your prescription payments.
• You have not received more than $250.00 in prescription
assistance from this agency over the past 6 months.
• You are over 55 years of age.
• You are a resident of Franklin County.
If YOU CAN ANSWER YES TO ALL SEVEN (7) OF THE ABOVE
STATEMENTS, YOU MAY QUALIFY FOR ASSISTANCE. PLEASE COLLECT
THE FOLLOWING DOCUMENTS AND CALL 457-2997 x 45 FOR AN APPOINTMENT
OR FOR ADDITIONAL INFORMATION.
• All current unfilled prescriptions
• For ALL members of your household
- Child support and/ or spousal support payment statements
for the last 3 months.
- Your most recent Social Security award letters and your
Social Security income statement.
- Statement(s) of wages for the past three (3) months from
your employer(s) on company letterhead or latest paycheck stub(s)
if it has a year to date (YTD) amount.
• Proof of age (birth certificate, driver’s
license, State of Ohio ID card, etc.
