Home-Outreach Program

The Home-Outreach Program was established to educate, assist and advocate on behalf of low income, uninsured and underinsured individuals who have COPD. During individual, in-home visits an experienced respiratory therapist, will assist you in learning more about how COPD affects your life, and how to maintain or improve your quality of life while dealing with this progressive disease.

The therapist also works with the Medical Supplies Coordinator and Better Breathing Club Coordinator to assure that each client receives individual attention to their particular needs. This may include ongoing support, equipment, prescription assistance, or referrals to other agencies for services such as in-home meals, or homemaking assistance.

To participate in the Home-Outreach Program, you must be:

  • Diagnosed with a chronic lung disease.
  • Over 60 years of age.
  • A resident of Franklin County.
  • Living independently (without skilled nursing care).
