POSITION TITLE:

Outreach Specialist Nurse: Case Manager for the Lung Health Clinic

RESPONSIBLE TO:

Lung Health Clinic Director

GENERAL DESCRIPTION:
The Outreach Specialist Nurse: Case Manager/Asthma Educator is responsible for assisting with daily clinic function and case management of the client under the direction of the clinic provider.  Our goal is to help improve the daily functioning of persons with asthma, COPD, and other lung-related problems. S/he is responsible for planning, implementing, and evaluating agency programs that respond to the psychological and educational needs of adults with the diagnosis of asthma and other lung-related problems. This Breathing Association staff member will assist the Director in the clinic and aboard the mobile medical unit (MMU) with direct patient care, spirometry testing, accurate and timely record-keeping, and reporting, discharge teaching, patient prescription assistance program applications, coordination and administration of vaccines as well as testing for COVID-19, referrals to and other medical/social referrals as needed.

POSITION TITLE:

Customer Service Representative (CSR)

RESPONSIBLE TO:

HEAP Office Manager

GENERAL DESCRIPTION:
Under the administrative direction of the HEAP Office Manager, this HEAP Hot Line CSR will provide advocacy, instruction, direction, and answer incoming calls from customers to cancel or confirm appointments, answer program questions, address complaints and escalate calls to management as necessary. In addition, this CSR will maintain the gateway of communication and information for all HEAP stakeholders.
